How do the health care providers decide which implementation guide (institutional or professional or dental) to follow?

For example, can a physical therapist submit a transaction based on the professional implementation guide or must it be the institutional implementation guide? What should home health providers use? Should an oral surgeon use the professional or dental implementation guide to submit a transaction?
